All participants attending any official RECF event must complete a Participant Release Form. This includes students, coaches, and mentors.

Who Should Complete the Form

  • Participants 18 or older: complete and sign the form yourself.
  • Participants under 18: a parent or guardian must complete and sign the form.

Required Team Information

Before completing the form, obtain the following from your coach:

  • Program name (e.g., ADC)
  • Team number/letter (e.g., 12345A)

Digital Release Form - Preferred Method

The digital release form is the preferred way to submit waivers. Once completed, the information is automatically uploaded to the team profile in RECF Events. This method ensures that the form is valid for the entire season and accessible at all competitions.

Option 1

  1. Team coach logs into RECFevents.org.
  2. From the left-hand menu, select Teams.
  3. Click on the desired team number.
  4. Select the Consent Forms tab.
  5. Click "Copy Link for Online Consent Form".
  6. Send the copied link to parents and guardians. 
  7. Ensure parents and guardians have the correct team number (including letter) as they will need this info to submit the form.

    1. Important: When entering the team number, be sure to include both the number and letter without any spaces or special characters.

    2. ✅ Correct: 1234A
      ❌ Incorrect: 1234 A, 1234-A

Option 2

  1. Share this link, digital release form, with parents and guardians.

  2. Instruct them to include the child's team number and letter on the form.

Important: When entering the team number, be sure to include both the number and letter without any spaces or special characters.

Paper Forms

While paper forms are still an option, they require manual processing and the coach will need to scan and upload to the team profile on RECFevents. Whenever possible, we strongly encourage the use of the digital form.

The team’s registered Primary, Secondary, and Financial Contacts will be able to upload participant consent forms to RECFevents.org. 

  1. Collect completed paper consent forms from students, parents/guardians, and mentors.
  2. Scan or take a high-quality photo of each form.
  3. Accepted file formats: .JPG, .PNG, or .PDF
  4. Save the files to your computer, clearly labeled for easy identification.
  5. Upload the files to RECFevents.org
    1. Log into RECFevents.org.
    2. Select Teams from the left hand menu.
    3. Select the team number.
    4. Select Consent Forms.
    5. Input first name and last Initial of the team member.
    6. Upload form.
    7. Repeat for additional team members.
